However, access to specialized eye care services can be limited for many patients, especially those living in rural or underserved areas. Ophthalmic telemedicine has the potential to improve access to eye care for all patients, but especially those with diabetes by connecting primary care providers with specialists for virtual consultations. By leveraging technology, telemedicine can provide patients with timely and convenient access to expert care, without the need for travel or prolonged wait times. Objective: The objective of this proposal is to implement ophthalmic telemedicine in primary care offices in South Florida, to improve the diagnosis and treatment of diabetes-related eye problems. Methodology: The implementation of ophthalmic telemedicine in primary care offices will involve the following steps: 1. Equipment and technology: The primary care office will be equipped with telemedicine technology, including high-quality cameras, lighting, and secure communication platforms to ensure optimal video quality and privacy. 2. Provider training: Providers will be trained on the use of the telemedicine equipment and software, as well as the protocols for conducting virtual consultations with specialists. 3. Integration with electronic health records (EHRs): The telemedicine system will be integrated with the primary care office's EHR system to ensure seamless transfer of patient information and continuity of care. 4. Virtual consultations: Patients with diabetes who are at risk for eye problems will be referred by their primary care provider for virtual consultations with an ophthalmologist. During the consultation, the specialist will review the patient's medical history, perform a visual acuity test, and assess the patient's eye health using the telemedicine equipment. 5.     Proof of Concept: One telehealth ophthalmology camera has already been implemented in a primary clinic in Hollywood, Florida with tremendous success. The camera was implemented in July 2022 at a Primary Care Clinic, located in Hollywood, Florida 33024. The adoption of the telehealth Ophthalmology camera was readily adopted by the primary care clinicians within the clinic as it provides a greater level of care and assessment to their patients as well as meets criteria for the Medicare Annual Wellness visit. Thus, the primary care clinicians are willing and able to be part of this initiative which is a critical and necessary to the success of this proposal. In fact, the initial proof of concept has been so successful, three additional primary care clinics in Broward Country are requesting telehealth cameras to be placed in their clinics as well. This proof of concept has been accomplished in one location, with success. However, opportunity exists to capture the effectiveness of this program in providing increased access to care across the underserved areas of South Florida and measure the outcomes discussed below with the funds provide by the grant award. Discussions with multiple other Primary Care Clinics in Miami-Dade, Broward and Palm Beach counties are currently ongoing. Proof of concept has already been established with one primary care clinic already utilizing the ophthalmic telemedicine with huge success. 3 additional clinics have already requested this to be implemented in their clinics. This speaks to the need and success of the program as well the growth strategy in the coming years.

    No current competitors exist for this specific plan. We are the only ophthalmic telemedicine platform that exists in the region. Optometrists and Ophthalmologists would be the only competition. However, the telemedicine implementation in primary care clinics allows for a more efficient and effective treatment and diagnosis of the patient's condition as well as expands access for those patients. Thus, the treatment plan can be initiated during the patient's primary care visit.
