I will have attorney fees of about $12,000, lease fees for office of about 90,000 (per year), and have to create office space and treatment rooms for the practice. I don't know how much that will be yet, but it is likely about $40,000 or more. EMR and billing related costs are not finalized, but likely about $30,000. Consulting fees for any needed training: $15,000. None of the above have any salary or money being paid to staff or providers. Annually for the current 3 employees (1 each- NP, RN, MA). that will be about $350,000 the first year minimum. I will also need to hire a part time NP to cover in case of emergency, as when we start, I will be the only provider.

Business Plan
I plan to open one location and plan to open 4 more in rural northern California to meet the needs of the population. I will be accepting both medicare and medi-cal, along with commercial insurances. I want to make a difference in mental health services in severely underserved areas. I am a psychiatric nurse practitioner with a degree in Business and was a formal financial planner. I know what it takes to start and grow a business. I will be moving out of my area (by Sacramento) to an area I have not made many contacts in yet, but plan to network to grow my business, as I have done in my current area. I decided to change into mental health care due to the incredible needs I saw once my nephew was diagnosed with schizoaffective disorder. He has remained largely untreated and goes in and out of inpatient units due to the lack of care in Sonoma county and with his current health system. I want to help that population as much as I can gain access to this needed speciality.

Self Identified Competition
There is only one clinic that provides spravato in Santa Rosa and does not take all the insurances. In the areas I plan on expanding, there are no providers. There is a dead zone that covers most of Northern California for this medication (1 Santa Rosa, 2 Redding, 1 Chico and 1 pending in Crescent City), so there is no access at all for many people suffering from treatment resistant depression and acute suicidal intent beyond the Bay Area or Sacramento Valley. I am planning on taking everyone. I know some will pay more and some less than my cost. I am going to use the ones I make more on to fund those who I don't. I plan on overpaying my staff and providers to make sure I can obtain the best talent possible and create an environment they will want to stay at by providing opportunities when we expand to additional areas. I also plan on being a teaching center to help teach those who want to learn. I will be hiring and teaching NP, RN and MA students. MDs are welcome as well, but often they already have places to go. It is the NP and RN programs that do not.
