No other group, that I am aware of offers our six programs under one umbrella. Other Chronic Care Companies only have one program. We have started with six programs (Chronic Care Management, Remote Patient Monitoring, Behavioral Health Integration, Transitional Care Management, Advanced Care Planning and Diabetes Self Management Training. We are working with physician groups to identify other needed programs to help the patients receive the care that they need.
