Growth is like change and it must occur. Patriot Healthcare Services was developed for the people. This particular organization just wants to give back to the people by providing relief to struggling family members and allowing them to be a father, brother, mother, daughter and spouse again rather than a caregiver. Furthermore, I would like to add on Services of skilled nursing to my organization and have CNA, LVN and RN bring healthcare to the homefront. Lastly, have the goal of bringing skilled therapy to the home for those with disabilities. Healthcare is changing and a new pattern and trend is emerging in an industry moving back to home health. Providers are adhering to taking care of patients only at facilities and RN case managers are now taking on the role of handling the care of patients when they leave the facility. However, a GAP exist. RN Case managers are struggling trying to find care once the patient has left the facility with limited network. Traditional routes put patients with disabilities in nursing homes as a step down process. However, Cost is the driving factor and patients want to keep cost low. Therefore patients result to care at home. Home healthcare can keep cost low and increase health recovery. Yet, caregivers are traditionally family members and burnout is traditionally achieved with this model.
