The plan for growth is broken into two phases over the next 12 months. Phase one will be the growth of the Family Medicine client list. Phase two will be the standup and growth of Regenerative Health services. Phase one is launch and grow the Family Medicine client list to fill five days per week. During phase one we will begin offering regenerative services. Phase one will complete when the Family Medicine client list fills a standard five-day work week. The completion of phase one will be the trigger to hire a second provider dedicated to Family Medicine. Phase two will begin between in conjunction with phase 1 with the goal of providing regenerative services which fill one day per week. Phase two will be considered complete when the regenerative services client lists fill a five-day work week. The completion of phase two will the second trigger to hire a third provider dedicated to Regenerative Health services. YEAR TWO At the completion of phase two Agape Health will continue to expand on its Family Medical Practice and Regenerative Health Services. Between year two and three Agape will begin setting the foundation for phase three which will be targeted for launch in year three. Agape Health will be transitioned to the holding company and three separate entities will be established under the parent company. Agape Family Medicine, Agape Regenerative Health & Agape Pediatrics. YEAR THREE TO FIVE In year three phase three will be executed. Phase three will consist of the launch of Agape Pediatrics. This will require phase one and two to be established and cash flowing as we will search out a specialized pediatrics MD or NP to launch this phase of growth. During year three to five Agape Health will continue to build out each division by providing a quality health care service and gaining in reputation as a choice provider for the Sheridan community. o Agape Health has completed an EHR implementation with an integrated billing platform that utilizes a built-in clearinghouse which reduces costs of filing claims. This system is cloud based and stores all records digitally reducing cost of office supplies and the need for additional employees. o We will be in network for three of the primary insurance carriers in the region and will also provide a self-pay option for those without insurance carriers. o Agape Health will have a partner dedicated to providing care and one partner focused on the management of the business. This will maximize the providers time to ensure quality care and maximize her time seeing patients. o The regenerative health care services will give Agape additional services in Sheridan allowing clients to receive treatment locally. • Opportunities: o Sheridan is an underserved community in provider to patient per capita. This provides Agape an opportunity to fit into the market and gain market share quickly. o As regenerative health services are almost non-existent in Sheridan this will allow Agape to brand itself as the choice provider in the region for regenerative health services which is a fast-growing segment in health care nationwide. o As Agape grows and its brand is built, we will be able to expand into other areas of health such as Pediatrics and provide another service which is underserved in Sheridan.

The market is seeing an increase in post COVID lag and autoimmune issues among both the vaccinated and unvaccinated population which has caused many to question the health care system. Patients have lost trust that their concerns are addressed by hospitals and desire a collaborative relationship with their primary care provider. Regenerative health care is growing at a rapid pace and in 2021 the United States market surpassed 30 billion dollars. Patients desire and are seeking out alternatives to pharmaceutical and medicinal therapies. The regenerative health services target the COVID lag and autoimmune issues the market is experiencing as well as many other chronic issues. Sheridan is an underserved community in health care and many travel to larger metros in order to access services such as would care, chronic pain treatment and regenerative health services to list a few. There are limited to no regenerative health services in PRP, IV therapies and Ozone/Prolozone therapies in or around the Sheridan, WY area.
