    Upon establishment in 2015/2016, Sequence To Health offered only primary care services. In 2018, I closed the practice to focus on finishing my Doctorate in Psychiatry/Mental Health. I reopened October 2019. The goal was to expand my knowledge so that I could offer services to patients with serious mental illnesses. Having both mental health and primary care needs met by the same Healthcare Provider promotes continuity of care; thereby, increasing Patient/Client satisfaction, decreasing incidence of polypharmacy, and subsequent hospitalization. We offer weekend hours; this prevents adults from missing work and children from missing school. In addition, we offer pain management without the use of opioid addictive medications via biomechanical stimulation. I would like to build on current progress and increase capital gain by offering a rehab retreat, within the next 6 months to 1 year. Reimbursement for services will be primarily self-pay. The insured patient will have the opportunity for reimbursement by their insurance company if they have coverage for such services under their plan. Investors should invest because I have a solution to a undeniable community problem. PROBLEM STATEMENT Patients with mental health problems such as generalized anxiety disorder (GAD) usually present first in the primary care setting (Kozlowski, Lusk, Melnyk, 2015). If GAD is not treated, the problem can escalate and lead to self-medicating activities with use of illicit drugs and alcohol abuse (Harris & Edlund, 2005). The sequence to restoring mental health is early recognition, diagnosis, and treatment. Generalized anxiety disorder can cause a wide spectrum of symptoms including irritable mood, attention deficit, extreme fatigue, insomnia, and muscle rigidity (National Institute of Mental health, 2016). Clinically, those with GAD often present with complaints of neck and back pain/tension associated with pronounced worry and apprehension related to daily stressors. Anxiety is one of the most prevalent mental illnesses that have an array of evidence based treatment modalities that are underutilized in primary care and other community-based practices (Mnookin, 2016). SOLUTION STATEMENT Cognitive behavior therapy (CBT) is a form of psychotherapy used to help individuals acknowledge that negative automatic thoughts lead to counterproductive behavior (American Psychological Association (APA), 2019). This form of therapy teaches the individual how to replace negative thoughts and behavior with positive coping skills. Likewise, activities designed to help induce relaxation such as yoga, meditation, and massage also provide benefits in treating anxiety and there are no threats of harmful side effects (Stein & Craske, 2017).     I am not aware of healthcare practices in my area that offer primary care, mental health care, and pain management services by one healthcare provider for the purpose of continuity of care in a holistic manner. I offer services that many healthcare offices do not under one location. I offer primary care services to include annual physicals, treatment of acute illnesses and chronic illnesses as well as treatment of mental illnesses with both therapy and medications when necessary.
