5by5 Performance Therapy 85-3893765 | Government Grant Application

5by5 Performance Therapy | Application Preview

5by5 Performance Therapy is seeking a $45,000 government grant to help cover overhead costs and sustain its vital services for veterans. The funding will enable the organization to stabilize financially and focus on transforming into a profitable entity. The long-term vision includes establishing a dedicated performance facility in Alaska tailored for special operations personnel, which would also support veterans by addressing their unique healthcare needs.

The business plan emphasizes securing investors and approval from the Department of Defense (DoD) to facilitate funding for lodging and transportation for veterans. 5by5 Performance Therapy has already demonstrated success with its model, which includes providing specialized care for various challenges veterans face, such as moral injury, mental health issues, nutrition, and family dynamics.

There is currently no direct competition offering the same specialized services, as 5by5 combines a small team of experienced clinicians who can mobilize for intensive week-long sessions followed by a month of telehealth consultations. This personalized approach is rooted in genuine care for clients, many of whom are veterans or have close ties to the veteran community. With over 50 successful client interventions, the organization has received strong testimonials indicating life-changing impacts.

By securing this grant, 5by5 Performance Therapy aims to continue its mission of saving lives while planning for future growth and expansion through the establishment of a dedicated performance facility.

  • Funding Usage

A grant of $45000 will allow 5by5 Performance Therapy to "get out of the hole" in terms of overhead business costs, and allow us to continue saving the lives of veterans without financial burden. I need to transition into making a profit in order to continue running this highly successul program. Ultimately, I'd like to build a brick and mortar facility here in Alaska to run an special operations specific performance institution, vetted by and funded by the DoD. Right now, I have 5-6 contract requests for next year, and $45,000 will help cover all the costs listed previously to keep us running through 2023.

  • Business Plan

Ultimately, I'd like to secure investors to build a brick and mortar performance facility for special operators, who have very unqiue medical and therapeutic needs. Once this is operational I'd love to open it up to Veterans. I'd like for this to be approved by the DoD, so that the DoD will pay for lodging and transportation costs. I've executed this performance model multiple times, collected research on the model, and many testimonials. This is an extremely successful model of healthcare that addresses the needs our veterans have right now: moral injury, operator syndrome, PTS and mental health, nutrition, family issues and so much more. Investors should assist because my model is already working- we are saving lives, but we're struggling financially being a newer business model (2 years).

  • Self Identified Competition

Currently, there is no other business that is doing what we are doing: a small team of highly experienced veteran/special operator specific populations who are able to mobilize for 1-week "reset weeks" combined with 4-weeks of telehealth. My business is different because we're already doing the work to save special operators and their family members. We are: - a small team of interdisciplinary clinicians (psych, OT, PT, nutrition) who work together on the care of each client - we are able to mobilize to a set location for 1-week durations - we combine concepts learned in person with 4-weeks of spouse and family-centered virtual care - we genuinely care about each of our clients and each of us are uniquely tied to the veteran population (veterans ourselves, spouses, etc.) - we have treated over 50 clients using this model and all clients have been incredibly happy, and many have expressed we've saved their lives - we know this model works- we want to grow it by building a performance facility in the next 5 years
