  • Funding Usage

    Funding will be applied to the expense of licensure and accreditation, and legal guidance to ensure infallible initiation of services. This is top priority and will ensure adequate income to further our core goals. Remaining funding will be utilized for daily operations, office space, refinement of electronic health record software with focus on HIPPA compliance and interoperability, livable and appropriate wages and benefits for providers and other employees, maintenance of secure and functional technology and services, and appropriate PPE (Personal Protective Equipment) for employees and clients.

  • Business Plan

    Within the next five years, we intend to transition to non-profit status to maximize the impact on our community. In addition to transitional care, and chronic care management via certified telehealth services, we will support the community through utilization of pharmacists for preventative care i.e., vaccinations, and awareness campaigns. We will utilize our trauma informed behavioral health professionals to support the community through guidance and counsel as related to current mental health concerns with the growing pandemic and impacts of isolation. We will expand our practice by integrating Occupational Therapy and client engagement specialists to maximize our comprehensive approach to client care. We plan to broaden our reach to include school system support, HIV/AIDS testing and education, and increasing access to adequate and appropriate care for members of the community across the lifespan. Our innovative approach to transitional care and chronic care management utilizes pharmacist expertise to provide medication management services and education to clients during traumatic health events, bridging the communication gaps between clients and each of their providers. Our services decrease hospital admissions and preventable readmissions, increase client safety and medication compliance, thus minimizing the cost of healthcare per individual case. In recent years, CMS has imposed penalties on hospitals with increased numbers of readmissions, further burdening the care system and increasing overall costs. This demonstrates the impact our program model will have on cost to care. Through trauma informed behavioral health services, we recognize and bridge further gaps by identifying and addressing specific individuals' needs based on their unique social, cultural, and community backgrounds. We intend to change the face of healthcare by focusing on maximizing client support and decreasing overall costs for providers, payors, and clients alike. We will help pave the way and support pharmacists in transitioning to provider status. We are a minority owned and operated business, competent in support for LBGTQIA+, and other socially determinate healthcare disparities. We work alongside existing community coalitions, guided by CMS initiatives, to maximize our positive influence on “The City Beautiful” and Central Florida.

    In the world of healthcare, ensuring high quality care is not a competition, and our utmost priority is the success of our clients and the conservation of funds throughout the continuum of care. Our multi-faceted approach focuses on existing limitations within the healthcare system with the goal of limiting preventable, adverse effects of clients transitioning between various levels of care. We cannot currently identify any competitors with similar pathways to these goals. While select hospitals and care facilities have designed or implemented client supportive and educational programs, they are limited during care transition due to the current structure of the healthcare system. Often, these programs focus on single conditions and require consistent client participation. Our practitioners will provide client-centered and evidence-based guidance and education, tailored to clients’ individual needs throughout the transition process. We support both physical and mental health conditions to minimize negative impacts on overall health and quality of life, while maintaining client dignity, and empowering individuals to drive their own health management.
