A New Moment Medical LLC 93-1803861 | Government Grant Application

A New Moment Medical LLC | Application Preview

A New Moment Medical LLC is seeking government grant funding to establish a medical practice that addresses significant health issues in their community, including substance abuse, obesity management, and chronic health conditions arising from obesity. The funding will be allocated for essential startup expenses such as lab equipment, utilities, office supplies, medical supplies, electronic medical records (EMR), advertising, and staff.

The business plan focuses on providing accessible health care in a community where such services are scarce. Currently, residents must travel over 30 miles for necessary treatments. The applicant, a Family Nurse Practitioner with experience in the local Urgent Care, plans to generate initial clientele through word-of-mouth and targeted advertising. Strategies include using flyers, billboards, community events, and referrals from other health care facilities to create awareness and build patient volume.

The funding request is backed by the applicant's personal connection to the health issues they aim to address. Their family history with obesity and the loss of a close friend to substance abuse have fueled a passion to improve community health. The applicant believes in the transformative power of health, emphasizing that weight loss can boost self-esteem and reduce chronic health risks, while also targeting the pervasive issue of substance abuse that affects families and community dynamics.

In terms of competition, Union County lacks providers offering comprehensive treatment for substance abuse and weight management. Existing facilities focus on primary care or mental health, but do not address the specific needs of this population. The applicant's established rapport with current patients and support from employers at the Urgent Care presents a unique advantage, enabling them to effectively promote their new services.

In summary, the applicant is motivated to fill the health care gap in their community by providing much-needed services and believes that with proper funding, they can create a positive impact on individual lives and the community as a whole.

  • Business Plan

I work in an Urgent Care in the same community that I am going to open my business. Substance abuse treatment, obesity management, and chronic health issues stemming from obesity are health concerns that are not being met in the community. Patients have to travel an excess of 30+ miles for needed treatment. I plan to start by word of mouth, which I have already started, and advertising through my current Urgent care center to build clientele. This will also allow me to add as needed services and the staff to cover growing volume. Advertising including flyers, bill boards, community events and referral from other health care facilities will help increase volume. I am a Family Nurse Practitioner working in a community that doesn’t offer needed health care and I feel that I can change that. I believe that in order to be healthy, you have to feel good about yourself which starts with being healthy including weight loss. Weight loss leads to better self esteem, attitude, lower blood pressure, lower cholesterol and lower blood sugar which improves chronic conditions. Substance abuse is prevalent in the community and I see many lives daily that are ruined. Children have parents addicted to drugs and alcohol and these substances are available but help to treat these conditions are not available. This has put a strain on families and the community as a whole. I want to change this and with the proper funding, I can do that. I come from a family that has fought obesity which caused chronic health issues lowered quality of life. I also lost my best friend that I grew up with to substance abuse. I saw first hand how it changed her and ruined her life. I am passionate about these health related concerns and I am motivated to change a county one person at a time that in hopes will cause a domino effect in the right direction.

  • Self Identified Competition

In Union County, there are no competitors. There are mental health counselors without substance abuse treatments. There are 2 main primary care facilities that no one can get an appointment at and they do not offer substance abuse treatment or weight loss. There is One hospital and one urgent care. Neither one of these offer the same services. As stated above, there are no competitors. I work in Union County at an Urgent Care and I have a good rapport with patients. I have come to understand what services are needed and not offered in this county. I also have a good rapport with my employers whom have agreed to let me refer myself and my services to patients that we serve.
